Financial eligibility
States evaluate income and countable resources. Some applicants may use a qualified income trust or another state-recognized pathway when income exceeds a program cap. Do not assume a strategy is available without state-specific confirmation.

Functional eligibility
Many HCBS programs require a level of care comparable to an institution. The assessment may examine activities of daily living, cognition, behavior, medical complexity, supervision, and risk.
Program eligibility
- State residency and citizenship or qualified immigration status
- Membership in the waiver's target population
- Availability of a waiver slot or managed-care enrollment
- A safe and cost-effective service plan
- Use of enrolled providers
Eligibility is not the same as coverage
A person can qualify financially but still need a functional assessment, program enrollment, and a provider able to deliver the authorized services.
